At Motion Matters, we welcome Medicare referrals as part of our commitment to providing accessible, high-quality care. All our practitioners are fully accredited with Medicare, ensuring you receive expert support tailored to your health needs.

If you have a chronic medical condition, you may be eligible for up to five rebated sessions per calendar year with allied health professionals under a Chronic Disease Management (CDM) Referral (previously called an Enhanced Primary Care (EPC) plan).
What Does This Mean For You?
With a CDM referral, Medicare provides a rebate of $60.35 per session for 1:1 services at Motion Matters. Unfortunately, the referral can not be used for our group classes. The rebate may be higher if you have exceeded the Medicare Safety Net for the year.
Am I Eligible?
You may qualify for a CDM plan if you meet the following criteria:
Chronic or Terminal Medical Condition: The condition must have been present for 6 months or longer.
Ongoing Multidisciplinary Care: Your treatment plan must involve your GP and at least two other medical or allied health practitioners.
Common Conditions Covered:
Diabetes (Type 1 & 2)
Kidney Disease
Cancer
Asthma
Chronic Musculoskeletal Conditions
Cardiovascular Disease
Stroke
How Does It Work?
The referral entitles you to up to five Medicare-rebated 1:1 sessions with allied health professionals in a calendar year. Referrals are valid for two years.
You can choose to allocate these sessions entirely to one discipline, such as Exercise Physiology, or split them between multiple providers. For example:
5 sessions with an Exercise Physiologist at Motion Matters
Or a combination, such as 2 sessions with an Exercise Physiologist, 2 with a Podiatrist and 1 with a Dietitian.
